The text, with signature lines, includes: To all whom it may concern, Be it known, that application having been made to The Grand Lodge of United States National Progressive Order of Odd Fellows, by the Lodge of BLANK for permission to open a New Branch at BLANK under the Title of BLANK Lodge No. BLANK. This Dispensation is hereby granted by the Grand Lodge of the United States National Progressive Order of Odd Fellows, to the above named Lodge to open such New Branch or Lodge accordingly. And that the same shall be enrolled, acknowledged and received as a Branch of the National Progressive Order of Odd Fellows. Now Therefore, the Grand Master of the above named Order, by the authority of the Grand Lodge, do hereby certify and declare the said Lodge to be a lawful Lodge of this Order. And that a Blond of Union be granted to the Members thereof, and the Society hereby binding themselves and the said Lodge in strong Friendship and Union together. Assuring them of the Protection of the Grand Lodge so long as the Laws of the Order shall be observed and kept. Given under our hands and seal this BLANK day of BLANK in the year of our Lord 18BLANK.
